I have had sufficient opportunity to read this agreement. I have read and understand this agreement and I agree to be bound by its terms.

The undersigned acknowledges the existence of certain inherent in this type of training and here by agrees to assume all risks himself. He further relieves Kim’s White Tiger Taekwondo, it’s management, assigned or contract instructors and his fellow students from any liability resulting from personal injury and/or loss of personal property. Occasional photo or videos of class or special events may be taken. Such materials may be used as advertising media. The undersigned waives claim to all model rights whether active or passive. One invite per friend. May cancel with 30 days of written notice.
